How to Become Appointed

Welcome to Group Health. We appreciate the opportunity to partner with you and we look forward to a productive relationship.

To begin your appointment process, please complete, sign, and return the documents below, along with a copy of your Washington license and proof of errors and omissions (E & O) insurance. Keep copies for your records.

Signing and returning these forms lets you act as an agent on behalf of Group Health Cooperative and Group Health Options, Inc. Please be sure that your name and address match your Office of the Insurance Commissioner (OIC) license information.

We'll contact you if any documents are missing or incomplete. Once we've received all required paperwork, we will file your application with the OIC.

Important note:
The producer appointment process takes an average of one to two weeks to complete. After your application is processed with the OIC, we will e-mail your appointment certificate to you. You're free to do business with us once your appointment is active on the OIC's licensing page.
